Zusammenfassung

BAYADA Home Health Care, Inc. seeks a compassionate Part-Time Physical Therapist to deliver in-home rehabilitation in Springfield, MO and surrounding areas.

You will provide one-on-one treatment to clients in their homes, helping them recover after injury, illness, or surgery. Responsibilities include conducting assessments, creating individualized care plans, educating families, and documenting progress with modern digital tools.

Qualifikationen

• Active and current PT license.
• One year of clinical experience preferred.
• Previous home health care experience welcomed, but not required. We will train you!
• Ability to travel locally to cases as assigned.

Aufgaben

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, gait, strength, balance, and safety assessments.
• Implement integumentary protective interventions to prevent wounds and promote wound repair.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continually assess and revise the PT care plan, and participate with nursing in the multidisciplinary care plan.
• Educate and instruct clients, family members, or other client representatives, in rehabilitative care and activities to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
