Zusammenfassung

Columbia River Home Health is seeking a compassionate Physical Therapist to provide in-home patient care in the Tri-Cities and Walla Walla areas of Washington. You will develop personalized care plans in collaboration with our interdisciplinary team and educate patients and caregivers on exercises and techniques.

Qualifications include Washington PT licensure (or pending), 1–2 years in PT with home health preferred, CPR, a valid driver’s license, and strong communication.

Qualifikationen

• Licensed to practice as a Physical Therapist in Washington State (or able to obtain before start date).
• One to two years of experience in physical therapy; home health experience preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Strong communication, critical thinking, and time management skills.

Aufgaben

• Deliver skilled physical therapy services to patients in their homes.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Educate patients and caregivers on techniques and home exercises.
• Complete evaluations, progress notes, and discharge summaries in a timely and accurate manner.
• Support care transitions and discharge planning.
