Qualifications

• Valid Physical Therapy License in California.
• Accepting New Graduates.

Responsibilities

• Conduct thorough home-based patient assessments and develop care plans.
• Supervise PTA and ensure adherence to the plan of care.
• Coordinate with care team for smooth transitions between services.
• Educate patients and caregivers and prepare for discharge as appropriate.
• Document clinical notes and progress within agency timelines.
• Participate in care conferences and quality improvement initiatives.
• Deliver skilled physical therapy interventions in home settings.
• Ensure compliance with physician orders and professional standards.
