Job summary

BAYADA Home Health Care is seeking a Physical Therapist to provide in-home care for adult and geriatric patients in Framingham, Natick, Ashland and surrounding towns. This role offers great benefits and a strong work/life balance, with options for full-time or part-time schedules.

Responsibilities include visiting clients at home, conducting assessments, creating and updating individualized care plans, educating families, and documenting care using a tablet.

Qualifications

• A current Massachusetts Physical Therapist license.
• New grads welcome; prior home health experience not required.

Responsibilities

• Perform diagnostic tests and measurements, such as mobility, joint range, transfer status, stability, ambulation, strength, endurance, balance, and safety assessments.
• Develop and implement individualized care plans, including manual therapy, gait training, balance and other interventions.
• Continuously assess and revise the Physical Therapy care plan and collaborate with nursing in the multidisciplinary team.
• Educate clients and family members in rehabilitative care and activities to promote health, safety and independence.
• Accurately document observations, interventions and evaluations using a touch pad tablet.

Full-Time or Part-Time Physical Therapist - Home Health

BAYADA Home Health Care is currently seeking a Full-Time or Part-Time Physical Therapist to provide exceptional care to adult and geriatric patients in their homes in Framingham, Natick, Ashland and surrounding towns. Prior home health experience is not required and new grads are encouraged to apply!!.

Responsibilities include:

• Making visits to clients in their homes
• Perform diagnostic tests and measurements, such as the mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing and safety assessments.
• Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance and other interventions.
• Continually assess and revise the Physical Therapy care plan and participate with nursing in the multidisciplinary care plan, as appropriate.
• Educate and instruct clients, family members or other client representatives, in rehabilitative care and activities necessary to promote the client's health, safety and independent living.
• Accurately document observations, interventions and evaluations pertaining to client care management and services provided, utilizing a touch pad tablet.

Qualifications include:

• A current license as a Physical Therapist in Massachusetts
