Iowa Home Care is hiring a Home Health Physical Therapist to perform in-home evaluations and develop individualized treatment plans in the Des Moines area. You will work with an interdisciplinary team to enhance mobility, safety, and independence for patients at home.

Role requires Iowa PT licensure, 2+ years of experience, and strong communication. Flexible scheduling and strong clinical support are offered by a locally owned agency.

Formación

• Licensed to practice as a Physical Therapist in Iowa.
• Minimum of two years of physical therapy experience; home health experience preferred.
• Strong communication, organizational, and problem-solving skills.
• Valid driver’s license with reliable transportation.

Responsabilidades

• Conduct comprehensive in-home PT evaluations and develop individualized treatment plans.
• Provide mobility, strength, balance, and safety-focused interventions.
• Educate patients and caregivers on home exercise programs and safety strategies.
• Collaborate with nurses, OTs, STs, social workers and case managers to coordinate care.
• Document visits, progress notes, and care plans in compliance with regulations.
• Recommend adaptive equipment and home modifications when appropriate.
