Job summary

Iowa Home Care is seeking a skilled Occupational Therapist to provide in-home evaluations and therapy in the Des Moines area. You will develop individualized treatment plans to improve daily living activities and independence, working with nurses, PTs, SLPs, and case managers to coordinate care.

Responsibilities include educating patients and caregivers, recommending adaptive equipment or home modifications, documenting progress, and adjusting plans as needed to ensure safe, functional outcomes

Qualifications

• Licensed to practice as an Occupational Therapist in Iowa.
• Minimum of two years of OT experience; home health experience preferred.
• Strong communication, organizational, and problem-solving skills.
• Valid driver’s license with reliable transportation.

Responsibilities

• Conduct comprehensive in-home occupational therapy evaluations.
• Develop and implement individualized treatment plans focused on ADLs and IADLs.
• Provide interventions to improve upper extremity strength, coordination, cognition, and independence.
• Educate patients on adaptive techniques for safety in daily activities.
• Recommend adaptive equipment and home modifications to improve safety and accessibility.
• Educate patients and caregivers on home programs and fall prevention.
• Collaborate with nurses, physical therapists, social workers, and case managers to coordinate care.
• Document visits, progress notes, and care plans in accordance with standards.
• Monitor progress and adjust plans as needed; communicate changes to the team.
