Qualifications

Responsibilities

• Complete PT assessments and home evaluations.
• Identify limitations in strength, ROM, balance, gait and mobility.
• Establish individualized treatment goals and plans of care.
• Provide therapeutic exercise and functional training.
• Teach home exercise programs for patients and caregivers.
• Coordinate services with nursing, OT, ST, social work and case management.
• Maintain accurate clinical records and documentation.
• Encourage safe participation and promote independence in daily activities.
