Job summary

AdventHealth in Florida seeks a licensed Physical Therapist to lead patient-specific home health care plans, reassess progress, educate patients and families, and coordinate therapy services in a home setting.

You will work with physicians and agency staff to adapt plans as needs change, mentor others, and ensure high-quality care. A Doctorate is preferred with 1+ year home health PT experience and active licensure.

Qualifications

• Must hold a valid Physical Therapist (PT) license.
• Minimum 1 year of clinical physical therapy experience.
• Experience with Medicare-certified home health agency preferred.
• Fluent in English; basic computer skills and MS Office.
• Education requirements per licensing standards.

Responsibilities

• Develops patient-specific care plans with realistic goals in consultation with patients, families, and physicians.
• Reassesses patients at appropriate intervals and documents progress.
• Instructs patients and caregivers on use of assistive devices and home programs.
• Documents treatments in medical records and supports staff development.
• Coordinates and directs care of home patients requiring physical therapy.
