Job summary

Logan Health Medical Center is seeking a Physical Therapist to plan, implement, and adjust individualized therapy programs based on comprehensive patient evaluations. Responsibilities

• Develops, implements, and revises individualized, goal-directed physical therapy plans through evaluation and ongoing assessment.
• Performs comprehensive assessments reflecting functional, physical, mental, and environmental status.
• Maintains updated plan of care with goals, interventions, and visit frequency orders.
• Delivers therapy services per established standards and department policies.
• Completes documentation timely according to payer guidelines and practice guidelines.
