Job summary

Family First Healthcare Services seeks a licensed Physical Therapist to provide high-quality home-based therapy to patients recovering from surgery, injury, illness, or functional limitations across Florida. You will assess needs, develop individualized treatment plans, and deliver hands-on interventions to reduce pain, restore mobility, and promote independence.

Qualifications

• Must have a Master’s or Doctorate in Physical Therapy from an accredited program.
• Must have successfully completed the NPTE.
• Active Florida Physical Therapist license required.
• APTA-accredited residency training or prior home health experience preferred.

Responsibilities

• Conduct comprehensive physical evaluations and functional assessments.
• Interpret patient histories, evaluations, and diagnostic findings to develop treatment plans.
• Provide therapeutic interventions including strengthening, gait, balance training, and manual therapy.
• Educate patients and families on home exercise programs, safe mobility, ergonomics, and injury prevention.
• Adjust treatment plans based on patient progress and evolving needs.
• Collaborate with nurses, physicians, and other healthcare professionals to coordinate care.
• Maintain accurate, timely, and compliant documentation in the EMR.
• Ensure adherence to federal, state, and agency regulatory standards.
• Stay current with evidence-based practices and new therapeutic approaches.
