Job summary

CHRISTUS Home Health in Corpus Christi, Texas is seeking a Physical Therapist for a home health role with a generous compensation package. You will assess functional status, develop and implement a plan of care, and provide interventions to improve mobility and comfort for patients in their homes.

This role requires current PT licensure in Texas, a valid driver's license, and the ability to work independently within a patient-centered team.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and PT needs; assists in plan of care.
• Provides PT services within scope of practice as defined by state laws and plan of care.
• Observes and reports patient response to treatment to the supervising nurse and/or physician.
• Tells and informs participating health-care team, patient, and family/caregivers about plan of care and progress.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops PT plan of care per state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
• Demonstrates ability to work with patients of diverse backgrounds.
