Job summary

Shands HomeCare in Gainesville, FL seeks a Physical Therapist to deliver one-on-one home-based therapy with clinical autonomy under the Rehab Director.

You will conduct evaluations, develop plans, educate patients and caregivers, and coordinate with a collaborative homecare team. Frequent travel to patient homes is required; mileage reimbursement provided.

Qualifications

• Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or Doctorate).
• Current Florida Physical Therapist license (in good standing).
• Home health experience preferred.
• Must pass AHCA Level 2 background screening.
• Frequent driver with daily travel and mileage reimbursement provided.

Responsibilities

• Deliver skilled physical therapy services in home settings.
• Perform comprehensive patient evaluations and develop individualized treatment plans.
• Establish measurable goals and track functional progress.
• Educate patients and caregivers to promote safety and independence.
• Supervise and support Physical Therapy Assistants (if assigned).
• Maintain accurate and timely clinical documentation.
• Collaborate with interdisciplinary healthcare teams.
