Job summary

All About Home Care Inc. in Houston, TX, is seeking a licensed Physical Therapist Assistant (PTA) to care for patients in the Southeast Greater Houston area, including League City. PRN PTAs for The Woodlands and Conroe are also sought. We document patient care using Kinnser software.

Qualifications

• Graduation from a two-year college level PTA program approved by the American Physical Therapy Association (APTA).
• Current license to practice as a PTA in Texas.
• Two years of Home Health experience preferred.

Responsibilities

• Understands and adheres to established policies and procedures.
• Provides physician-prescribed physical therapy under a plan of care established by a PT.
• Improves or minimizes residual physical disabilities of patients.
• Returns the individual to optimum and productive level within patient’s capabilities.
• Performs all skilled procedures as ordered by physician and according to the plan of care established by a PT.
• Participates with other health care personnel in patient care planning.
• Takes initial history and performs initial evaluation.
• Instructs patients and family in home programs and activities of daily living.
• Participates in in-service programs and presents as assigned.
• Participates in performance improvement activities as assigned.
• Attends patient care conferences as scheduled.
• Prepares medical records, progress notes, and updates care plans for each patient visit.

Job Description:

All About Home Care is a Houston-based home health care agency. We’ve been serving Houston and the surrounding communities since 2004, providing quality home health services. We are currently looking for Physical Therapist Assistant (PTA) to help care for patients in the Southeast part of the Greater Houston including areas such as Baytown, Pasadena, La Porte, Pearland, Friendswood, League City, and Texas City. We are also seeking PRN PTAs for The Woodlands and Conroe areas. We use Kinnser software to document patient care.

Qualifications:

• Graduate from a two-year college level program approved by the American Physical Therapy Association


• Currently licensed in the state of Texas


• Two year of experience in Home Health preferred

Responsibilities:

• Consults with PT regarding change in treatment


• Instructs patients and family/significant others in home programs and activities of daily living


• Participates in mandatory in-service programs and presents in-service programs as assigned


• Participates in performance improvement activities as assigned


• Attends all patient care conference as scheduled


• Prepares medical records, progress notes, and updates care plans for each patient visit in a timely manner as per Agency
