Job summary

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current CPR certification or ability to obtain within 90 days of hire.
• Ability to perform physical tasks required for patient care, including lifting and transfers.

Responsibilities

• Follow state laws governing delivery of physical therapy in home care.
• Follow treatment plan defined by supervising PT; perform only qualifying procedures.
• Instruct patients and caregivers in exercises, gait, ADLs, and mobility.
• Observe and report patient responses; coordinate with the care team.
