Job summary

Remedy Home Health Care is hiring a Physical Therapist Assistant (PTA) for the East LA Territory. You will deliver skilled in-home therapy under a supervising Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility for seniors.

The role offers per diem flexibility with autonomy to manage your patient schedule and participate in a collaborative clinical team. Per visit compensation and benefits structure apply to eligible employees.

Qualifications

• Active California PTA license is required.
• Current CPR certification (AHA) is required.
• Minimum 1 year of clinical experience, home health preferred.
• Strong interest in home health and senior care.
• Excellent communication and organizational skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide in-home physical therapy services following Plan of Care.
• Assist patients with therapeutic exercises and mobility training.
• Support senior care patients in improving strength and balance.
• Monitor, document, and report patient progress to supervising PT.
• Educate patients and caregivers on home exercise programs and safety.
• Collaborate with physicians, RNs, and the care team; complete EMR documentation.

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities:

• Provide in-home physical therapy services following the established Plan of Care
• Assist patients with therapeutic exercises, mobility training, and rehabilitation programs
• Support senior care patients in improving strength, balance, and independence
• Monitor, document, and report patient progress to the supervising Physical Therapist
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Collaborate with physicians, RNs, and the interdisciplinary care team
• Complete documentation using WellSky Kinnser EMR

Qualifications:

• Active California Physical Therapist Assistant (PTA) license
• Current CPR certification (AHA required)
• Minimum 1 year of clinical experience (home health experience preferred)
• Strong interest in home health, rehab, and senior care
• Excellent communication and organizational skills
• Valid driver’s license, auto insurance, and reliable transportation
