Remedy Home Health Care is hiring a Physical Therapist Assistant (PTA) to join our home health team in East LA Territory (Bellflower, Downey, Monterey Park area). This per diem role offers flexible schedules and the chance to deliver compassionate, one-on-one rehab in patients' homes.

As a PTA, you’ll work under a licensed Physical Therapist to support rehabilitation, mobility training, fall prevention, and independence for seniors.

Competenze

• Active California PTA license and CPR (AHA).
• Minimum 1 year of clinical/home health experience.
• Valid driver’s license and reliable transportation.
• Excellent communication and organizational skills.

Mansioni

• Provide in-home PTA services under the supervision of a licensed Physical Therapist.
• Deliver therapy, exercises, mobility training, and rehabilitation programs.
• Monitor and report patient progress to the supervising Physical Therapist.
• Educate patients and caregivers on home exercise programs, safety, and fall prevention.

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities:

• Provide in-home physical therapy services following the established Plan of Care
• Assist patients with therapeutic exercises, mobility training, and rehabilitation programs
• Support senior care patients in improving strength, balance, and independence
• Monitor, document, and report patient progress to the supervising Physical Therapist
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Collaborate with physicians, RNs, and the interdisciplinary care team
• Complete documentation using WellSky Kinnser EMR

Qualifications:

• Active California Physical Therapist Assistant (PTA) license
• Current CPR certification (AHA required)
• Minimum 1 year of clinical experience (home health experience preferred)
• Strong interest in home health, rehab, and senior care
• Excellent communication and organizational skills
• Valid driver’s license, auto insurance, and reliable transportation
