Job summary

Remedy Home Health Care is actively seeking a qualified Physical Therapist Assistant (PTA) to deliver one-on-one in-home therapy in the East LA Territory. This per diem role offers flexible scheduling and the opportunity to support senior patients in their homes, focusing on rehab, fall prevention, and functional mobility.

You will work under the supervision of a licensed Physical Therapist, documenting progress and collaborating with a multidisciplinary team to optimize patient outcomes.

Qualifications

• Active California PTA license.
• Current CPR certification (AHA required).
• Minimum 1 year of clinical experience (home health preferred).

Responsibilities

• Provide in-home physical therapy services following the Plan of Care.
• Assist patients with therapeutic exercises and mobility training.
• Support senior care patients in improving strength, balance and independence.
• Document progress and report to supervising PT.
• Educate patients and caregivers on home exercise programs and safety.

Your Role as a Home Health PTA (Physical Therapist Assistant)

As a Physical Therapist Assistant in home health, you will deliver skilled therapy services under the supervision of a licensed Physical Therapist, focusing on rehabilitation, fall prevention, and functional mobility.

Key Responsibilities

• Provide in-home physical therapy services following the established Plan of Care
• Assist patients with therapeutic exercises, mobility training and rehabilitation programs
• Support senior care patients in improving strength, balance, and independence
• Monitor, document, and report patient progress to the supervising Physical Therapist
• Educate patients and caregivers on home exercise programs, safety, and fall prevention
• Collaborate with physicians, RNs, and the interdisciplinary care team
• Complete documentation using WellSky Kinnser EMR

Qualifications

• Active California Physical Therapist Assistant (PTA) license
• Current CPR certification (AHA required)
• Minimum 1 year of clinical experience (home health experience preferred)
• Strong interest in home health, rehab, and senior care
• Excellent communication and organizational skills
• Valid driver's license, auto insurance, and reliable transportation

$55—$70 USD
