Job summary

Elara Caring is hiring a Physical Therapist Assistant to join our home health team. You will provide direct physical therapy services in patients’ homes, following the Plan of Care and physician orders, under supervision of a PT.

Travel within the service area is required, and you must maintain an eligible PTA license and vehicle. This role offers competitive compensation and a robust benefits package, with opportunities for professional growth and advancement within a mission-driven home care

Qualifications

• Graduate of an accredited PTA program approved for licensure.
• Current, unrestricted PTA license valid for state of work.
• 6+ months experience as a PTA is preferred.
• Able and willing to travel within branch/office coverage area.
• Must have a dependable vehicle and valid driver’s license.

Responsibilities

• Promotes Elara Caring's philosophy, mission and policies to ensure quality of care.
• Performs direct physical therapy services per standards of practice and physician orders.
• Follows Plan of Care as established by Physical Therapist and reports changes.
• Adheres to the State Practice Act Guidelines.
• Verifies the Plan of Care prior to each visit and provides care per physician orders.
