Elara Caring is hiring a Physical Therapist Assistant to join our home health care team. You will deliver direct physical therapy services, under the supervision of a PT, in patients’ homes across a broad service area. The role emphasizes quality care, patient education, and adherence to the Plan of Care.

Qualifications

• Graduate of an accredited physical therapy assistant program approved by the American Therapy Association.
• Current, unrestricted PTA license valid for the state of work.
• 6+ months experience as a Physical Therapist Assistant preferred.
• Able and willing to travel within branch/office coverage area.
• Must have dependable vehicle, valid driver’s license, and current auto insurance.

Responsibilities

• Promotes Elara Caring’s philosophy and policies to ensure quality of care.
• Performs direct physical therapy services per physician orders and Plan of Care.
• Follows Plan of Care under supervision of Physical Therapist and reports status changes.
• Verifies Plan of Care prior to each visit and provides care per physician’s orders and guidelines.
• Instructs patients and families in use of prosthetic/orthotic devices and home therapy program.
• Handles patient transfers and moves. Works in a potentially high-stress environment.
• Reports to Branch Director, Clinical Manager or designee.
