Affinity Home Health Care Inc. seeks a Physical Therapist Assistant (In-Home PRN) to provide compassionate, patient-centered PT assistance in clients’ homes under the supervision of a licensed Physical Therapist. You will implement treatment plans, assist with therapeutic exercises, and document patient progress.

Role emphasizes safety, empathy, and clear communication to support therapeutic outcomes and client dignity in home settings.

Physical Therapist Assistant (In-Home PRN)

Provide compassionate, patient-centered physical therapy assistance in clients’ homes for elderly, chronically ill, and disabled individuals. Work under the direction of a licensed Physical Therapist to implement treatment plans and support therapeutic outcomes.

• Assist with therapeutic exercises, mobility, and balance training to improve function.
• Use adaptive equipment and techniques to enhance safety and independence.
• Follow established treatment plans and clinical guidance from the supervising Physical Therapist.
• Document patient visits, progress, and relevant observations.
• Monitor changes in patient condition and communicate updates to the care team.
• Deliver care with a strong focus on quality, teamwork, and client dignity.

Qualifications & Skills

• Physical Therapist Assistant license/credentials as required by law.
