Affinity Home Health Care Inc. is seeking a licensed Physical Therapist Assistant to provide compassionate in-home rehabilitation for older adults and individuals with chronic or disabling conditions.

The role involves following a licensed Physical Therapist's plan, guiding patients through therapeutic exercises, and documenting progress and observations in the home setting. Collaboration with caregivers and the clinical team is essential to maintain care continuity and safety.

Competenze

• Completion of an accredited Physical Therapist Assistant program.
• Licensure/registration as required in the applicable jurisdiction.
• Strong interpersonal skills, empathy, and patient-centered communication.
• Ability to follow clinical direction, observe changes, and maintain accurate documentation.
• Comfort working in home environments and supporting clients with mobility limitations.
