Job summary

UnitedHealth Group is seeking a Physical Therapist for home health, offering flexible scheduling and a 4-day work week. The role involves assessing patients, developing treatment plans, and delivering interventions to improve mobility and well-being, while coordinating with a care team.

Responsibilities include evaluating functional status, documenting progress, educating families, and ensuring care aligns with state guidelines.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and access to reliable transportation or public transit.
• Preferred CPR certification (or ability to complete within 90 days).

Responsibilities

• Evaluate patient's functional status and PT needs; assist physician with plan of care.
• Provide PT services within scope of practice and plan of care, coordinating with care team.
• Observe and report patient responses and changes to supervisor/physician.
• Treat patients to relieve pain, restore function, and maintain max functioning.
• Educate patient and family about plan of care, limitations, and progress toward goals.
• Document progress notes after each visit and submit per policy.
• Develop PT plan of care within state guidelines and report outcomes.
• Determine visit frequency and adjust care based on productivity and goals.
