Job summary

Monticello, IA area seeks a Physical Therapist to provide home health services under physician orders and agency policies. Work to regain independence and improve quality of life with personalized rehab plans.

The role covers patient assessments, therauptic interventions, caregiver education, and collaboration with care teams. Travel to client homes is required; a $25,000 sign-on bonus is offered.

Qualifications

• Degree from an APTA-accredited Physical Therapy program.
• Active state license to practice as a Physical Therapist.
• 1+ year of experience or completion of a 6–8 week preceptorship.
• Home health experience and Medicare knowledge preferred.
• Valid driver’s license and reliable transportation.
• Strong organizational and time-management skills.

Responsibilities

• Conduct comprehensive assessments and develop individualized treatment plans.
• Deliver therapeutic interventions, including manual therapy, modalities, and exercise programs.
• Educate clients, families, and caregivers on therapy techniques and home programs.
• Supervise Physical Therapist Assistants (PTAs) and Home Health Aides (as permitted).
• Collaborate with interdisciplinary teams and participate in care conferences.
• Monitor progress, adjust treatment plans, and communicate changes to physicians.
• Maintain accurate, timely documentation in compliance with regulations.
