Job summary

Above and Beyond Home Health Care and Hospice Agency, a Trinity Health Company, seeks a Physical Therapist to deliver in-home, evidence-based care in the Monticello, IA area serving nearby communities.

Qualifications

• Must have degree from an APTA-accredited Physical Therapy program.
• Active state license to practice as a Physical Therapist.
• 1+ year of experience or completion of a 6-8 week preceptorship.
• Home health experience and knowledge of Medicare Conditions of Participation preferred.
• Valid driver’s license and reliable transportation.
• Strong organizational and time-management skills.

Responsibilities

• Conduct comprehensive assessments and develop individualized treatment plans.
• Deliver therapeutic interventions, including manual therapy, modalities, and exercise programs.
• Educate clients, families, and caregivers on therapy techniques and home programs.
• Supervise Physical Therapist Assistants (PTAs) and Home Health Aides (as permitted).
• Collaborate with interdisciplinary teams and participate in care conferences.
• Monitor progress, adjust treatment plans, and communicate changes to physicians.
• Maintain accurate, timely documentation in compliance with regulations.

Your Role

As a Physical Therapist, you’ll deliver professional, evidence-based care in accordance with physician orders, state regulations, and agency policies. You’ll help clients regain independence and improve their quality of life through personalized rehabilitation plans.
