Job summary

Above & Beyond Home Health Care and Hospice Agency, a Trinity Health Company, seeks a Physical Therapist to deliver professional, evidence-based care in patients' homes across the Monticello area.

You will assess, plan, and implement personalized rehabilitation programs, supervise aides where allowed, and collaborate with physicians and care teams to improve mobility and quality of life. A $25,000 sign-on bonus is offered.

Qualifications

• Degree from an APTA-accredited Physical Therapy program.
• Active state license to practice as a Physical Therapist.
• 1+ year of experience or completion of a 6–8 week preceptorship.
• Home health experience and knowledge of Medicare Conditions of Participation preferred.
• Valid driver’s license and reliable transportation.
• Strong organizational and time-management skills.

Responsibilities

• Conduct comprehensive assessments and develop individualized treatment plans.
• Deliver therapeutic interventions, including manual therapy, modalities, and exercise programs.
• Educate clients, families, and caregivers on therapy techniques and home programs.
• Supervise Physical Therapist Assistants (PTAs) and Home Health Aides (as permitted).
• Collaborate with interdisciplinary teams and participate in care conferences.
• Monitor progress, adjust treatment plans, and communicate changes to physicians.
• Maintain accurate, timely documentation in compliance with regulations.
