Qualifications

• Valid Physical Therapist license for the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• At least 1 year of related PT experience in home health care; mentoring if less than 1 year.

Responsibilities

• Help patients maintain, restore and improve movement and function.
• Perform reexaminations every 30 days minimum and adjust interventions.
• Discharge planning and other duties as assigned.
