Job summary

Optum's DFW Home Health is seeking a licensed Physical Therapist to conduct assessments in patients' homes, determine care plans, and deliver interventions aimed at improving mobility and comfort. You will monitor progress and coordinate with the care team to ensure quality outcomes.

Responsibilities include implementing treatment plans, educating patients and families, and documenting progress within state guidelines and policy. A rewarding role in a supportive, growth-oriented environment.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Assists physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws and plan of care.
• Observes and reports patient response to treatment to supervising nurse/physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs patient and family/caregivers regarding plan of care and progress.
• Visits patients according to Plan of Treatment and submits progress notes.
• Completes all evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated frequency of care.
