Job summary

Optum is seeking a dedicated Physical Therapist to join our home health team. You will perform assessments, develop and implement treatment plans, and coordinate care with the health team to improve patient well‑being in various counties within the service area.

Responsibilities include evaluating functional status, delivering therapy interventions, and documenting progress while ensuring compliance with state guidelines. A valid PT license and reliable transportation are required.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Preferred CPR certification (within 90 days of hire)

Responsibilities

• Evaluate patient functional status and therapy needs; assist in developing the plan of care.
• Provide PT services within scope of practice and plan of care.
• Observe and report patient progress to supervising nurse/physician.
• Treat patients to relieve pain and restore function.
• Instruct patient and family about plan of care and progress.
• Document visits and progress notes per policy.
• Develop PT plan of care within state guidelines.
• Report outcomes and anticipated frequency of care.
• Function respectfully in diverse home settings.
