Qualifications

• Two (2) years of experience in adult physical therapy, home health experience preferred.
• Physical therapy skills per competency checklist.

Responsibilities

• Perform physical therapy evaluation and treatment based on physician orders.
• Establish measurable goals and develop treatment plans for patient independence.
• Maintain communication among treatment team and family members.
• Supervise Physical Therapist Assistants and Aides.
• Maintain documentation of services for billing.

Graduate of a CAPTE approved Physical Therapist Education program
