Job summary

ASSISTED HOME HEALTH INC is seeking compassionate physical therapists to provide home-based physical therapy across patient locations in Florida. The role requires a Florida-licensed PT with a Bachelor's or Master's in Physical Therapy and at least one year of healthcare experience.

A valid driver's license and ability to spend ~20% of the day driving are essential. You will assess, document, and coordinate with physicians and families, supervise a PTA, and participate in required meetings,

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
• Florida licensed Physical Therapist.
• Minimum of one year healthcare experience.
• Current driver’s license; ability to drive ~20% of day.
• Commitment to deadlines for charting and time management.

Responsibilities

• Provide physical therapy as prescribed, safely in the home, and assist physician in evaluating patients.
• Observe and record activities and findings in the clinical record.
• Report to the physician the patient’s response to treatment and changes in condition.
• Instruct patient and family in care and use of physical therapy devices.
• Instruct other health team personnel and family members in phases of therapy.
• Instruct the family on the patient’s total physical therapy program.
• Supervises the Physical Therapy Assistant.
• Participate in case conferences, utilization reviews, orientations, in-services and CE meetings.

Essential Responsibilities and Duties:

(Note: Other Duties May Be Assigned)

• To provide physical therapy as prescribed by a physician which can be safely provided in the home and assisting the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional abilities test.
• To observe and record activities and findings in the clinical record
• Report to the physician the patient’s reaction to treatment and any changes in the patient’s condition.
• To instruct the patient and family in care and use of physical therapy devices.
• To instruct other health team personnel including, when appropriate, home health aides and family members in certain phases of physical therapy with which they may work with the patient.
• To instruct the family on the patient’s total physical therapy program.
• Supervises the Physical Therapy Assistant
• Participates in all required case conferences, utilization reviews, orientations, in-services and continuing education meetings.
