ASSISTED HOME HEALTH INC in Florida is seeking compassionate physical therapists to provide home-based PT services. You will evaluate patients, design treatment plans, and collaborate with physicians to improve mobility and daily functioning in the patient’s home environment.

Requirements include a Bachelor’s or Master’s degree in Physical Therapy, a Florida PT license, at least one year of healthcare experience, and a valid driver’s license with about 20% travel to patient locations.

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited college.
• Licensed Physical Therapist issued by Florida.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of day driving to/from patient locations.
• Commitment to meeting critical deadlines for charting and self-managing time.

Responsabilités

• Provide physical therapy as prescribed by a physician in home settings and assist the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional abilities tests.
• Observe and record activities and findings in the clinical record.
• Report to the physician the patient’s reaction to treatment and any changes in the patient’s condition.
• Instruct the patient and family in care and use of physical therapy devices.
• Instruct other health team personnel including home health aides and family members in phases of physical therapy with the patient.
• Instruct the family on the patient’s total physical therapy program.
• Supervises the Physical Therapy Assistant.
• Participates in case conferences, utilization reviews, orientations, in-services and continuing education meetings.

Essential Responsibilities and Duties:

• To provide physical therapy as prescribed by a physician which can be safely provided in the home and assisting the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional abilities test.
• To observe and record activities and findings in the clinical record
• Report to the physician the patient’s reaction to treatment and any changes in the patient’s condition.
• To instruct the patient and family in care and use of physical therapy devices.
• To instruct other health team personnel including, when appropriate, home health aides and family members in certain phases of physical therapy with which they may work with the patient.
• To instruct the family on the patient’s total physical therapy program.
• Supervises the Physical Therapy Assistant
• Participates in all required case conferences, utilization reviews, orientations, in-services and continuing education meetings.
