Domnovate is seeking a licensed physical therapist to deliver home-health and hospice therapy across patient homes in Newport, VT. You will use OASIS to shape goals and work closely with a care team to guide next steps in care.

This 13-week W-2 contract involves Monday–Friday, 8 a.m.–5 p.m. with 32 guaranteed hours per week planned for a 40-hour week, starting Oct 28, 2026, with travel between homes required.

Qualifications

• Active Vermont physical therapist license.
• At least two years of experience.
• OASIS, home-health and hospice experience required.
• Experience traveling between home visits and adapting the daily visit schedule.

Total Compensation: $49.12–$52.39/hour*

Pay and Benefits

Taxable W-2 hourly pay: $20.00–$23.27.

For eligible travelers, total compensation may include both taxable W-2 wages and separate travel reimbursements under Domnovate Healthcare’s travel reimbursement policy.
