Job summary

Trilogy Home Healthcare, a Humana company, is hiring Physical Therapists to provide one-on-one PT services in patients’ homes in the Lakeland, FL area. Full-time opportunities offer a salary range of $100,000–$120,000 per year with PRN visits also available.

Qualifications

• Graduate of an accredited PT program.
• Current Florida PT license.
• Valid Florida driver’s license, active vehicle insurance, and clear MVR.
• 1+ year of experience; Home Health experience preferred.

• Provide one-on-one physical therapy services in a home health setting.
• Help patients improve mobility, strength, and balance.
• Set and manage care goals to promote recovery and independence.
• Evaluate the home environment for safety and mobility needs.

Education

Graduate of an accredited PT program

Florida PT license

Valid Florida driver's license

Active vehicle insurance
