Job summary

Trilogy Home Healthcare is seeking a dedicated Physical Therapist to provide one-on-one home health visits in the Davenport and Winter Haven area. You will evaluate patients, create care plans, and guide recovery in the comfort of their homes.

Qualifications

• Graduate of an accredited PT program.
• Current Florida PT license.
• Valid Florida driver's license, active vehicle insurance, and clean MVR.
• 1+ year of experience; Home Health experience preferred.
• Sound judgment and passion for patient care and outcomes.

PRN $75 - $120/visit
