Job summary

Trilogy Home Healthcare Tampa Office is seeking a Physical Therapist (PT) to perform home health visits in Davenport and Winter Haven areas. Full-time role offering a salary range of $100,000–$120,000 per year, with PRN visits also available.

You will provide one-on-one PT services, help patients improve mobility and balance, set care goals, and assess home safety to promote recovery and independence. Join a collaborative team with strong leadership and growth opportunities.

Qualifications

• Graduate of an accredited PT program.
• Current Florida PT license.
• Valid Florida driver's license and active vehicle insurance.
• Clear MVR.
• Minimum 1 year of home health experience.

Responsibilities

• Help patients improve mobility, strength, and balance.
• Set and manage care goals that promote recovery and independence.
