Elara Caring seeks a Home Health Physical Therapist to join our team in Harker Heights, TX, offering full-time, Monday–Friday 8:00 a.m.–5:00 p.m. In this field-based role, you’ll deliver skilled PT in patients’ homes, develop treatment plans, and coordinate with a multidisciplinary team.

You’ll need graduation from an APTA-approved PT program, an active Texas PT license, and at least one year of healthcare facility experience. A valid driver’s license or reliable transport is required.

Qualifications

• Graduation from an APTA-approved physical therapy program.
• A current, unrestricted Texas PT license.
• At least one year of experience in a healthcare facility.
• A valid driver’s license and acceptable driving record, or reliable transportation between patient homes and work locations.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including patient positioning, transfers, and moving equipment.

Responsibilities

• Evaluating functional needs and creating individualized treatment plans.
• Providing skilled physical therapy in the home.
• Tracking progress and adjusting care as needs change.
• Teaching patients and caregivers strategies to support recovery between visits.
• Coordinating care with physicians and other healthcare professionals.
• Completing timely, accurate clinical documentation.
• Supervising therapy assistants and support team members as appropriate.

Education

Graduation from an APTA-approved physical therapy program

Current Texas PT license (unrestricted)

At least 1 year of healthcare facility experience

Valid driver’s license or reliable transportation

Ability to frequently lift up to 50 pounds
