Job summary

CenterWell Home Health in Fort Mill, SC seeks a Home Health Physical Therapist to help patients regain strength, mobility and independence in their homes, reporting to the Clinical Manager.

You will work with a patient-centered care team, perform comprehensive evaluations, and develop rehab plans aligned with physician orders. LP licensure and CPR are required, with a focus on teamwork and exceptional patient care.

Responsibilities

Job description

As a Home Health Physical Therapist, You Will

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes, to attain highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with physician's Plan of Treatment and the goals of the patient team.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

Required Qualifications

• Current and unrestricted Physical Therapy license
• Current CPR certification
• Experience collaborating with a team of healthcare professionals

Preferred Qualifications

• One year of experience as a Physical Therapist

Pay Per Visit/Unit Range

$59.00 - $83.00 pay per visit/unit

This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Driving Statement

This role is part of Humana's driver safety program and therefore requires an individual to have a valid state driver's license and to maintain personal vehicle liability insurance. Individual must carry vehicle insurance in accordance with their residing state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage or whichever is higher.
