Job summary

The VIOS Clinic in Missouri is seeking a Licensed Physical Therapist for in-home health visits. You will assess patients, develop individualized treatment plans, and deliver evidence-based interventions to improve mobility and independence.

Travel to patients' homes will be required, with documentation in the EMR. You will collaborate with nursing staff and physicians to coordinate care, educate patients and caregivers on goals and home exercises, and monitor progress to adjust treatment plans

Qualifications

• Licensed Physical Therapist with strong clinical assessment and treatment planning skills.
• Ability to deliver patient-centered, evidence-based care.
• Strong communication and collaboration skills.
• Comfort working independently in a home health setting.
• Proficiency with electronic medical records (EMR) documentation.
