Job summary

The VIOS Clinic is seeking a licensed physical therapist to provide skilled services to home health patients. You will assess patient needs, develop and adapt treatment plans, and deliver interventions aimed at improving mobility, strength, balance, and quality of life.

In this role, you will monitor responses, document outcomes, and collaborate with an interdisciplinary care team to ensure comprehensive patient care and compliant documentation.

Qualifications

• Licensed physical therapist with current license.
• Experience in home health beneficial.
• Strong communication and teamwork skills.
• Knowledge of clinical compliance requirements.
