Job summary

All Seniors Foundation is seeking a Home Health Pharmacist in Los Angeles, CA to review medications for elderly patients and coordinate with care teams. You’ll identify drug interactions, optimize regimens, and educate patients and caregivers in their home care settings.

The role emphasizes collaboration with physicians, nurses, and other clinicians, ensuring safe, effective therapy and adherence across home-based services. A strong foundation in geriatrics and EHR proficiency is required.

Qualifications
