Job summary

PruittHealth is seeking a skilled Occupational Therapist (OT) for Home Health in Georgia. You’ll provide evaluative, consultative, and therapeutic interventions to patients in their homes, collaborating with a multidisciplinary team to coordinate care.

Qualifications

• Must hold a valid and unrestricted professional license in state of practice.
• Graduate of ACOTE accredited OT program.
• If foreign-trained, the program must be deemed substantially equivalent to an Occupational Therapist entry level education in the U.S. by a credentials evaluation organization approved by the AOTA.

Responsibilities

• Assess, monitor, document, and report patient health status using required documents and timelines.
• Collaborate with interdisciplinary team to develop, integrate, and manage the plan of care; refer to other disciplines as needed.
• Demonstrate competency in home health care and patient care skills relevant to the role.
• Ensure timely, accurate documentation (OASIS, SOC/Admissions, Recertifications, ABNs, visits, orders, care coordination).
• Maintain compliance with safety regulations and agency guidelines, and participate in required trainings and meetings.
