Job summary

Qualifications

• Must hold a valid, unrestricted professional license in state of practice.
• Graduate of ACOTE-accredited school.
• If foreign-trained, program must be deemed substantially equivalent to U.S. OT entry-level education by a credentials evaluation organization approved by AOTA.

Responsibilities

• Assess, monitor, document and report patient health progress using required documentation and clinician technology.
• Collaborate with interdisciplinary team and physician to develop and manage the plan of care.
• Demonstrate competency in home health care and specific patient care skills.
• Maintain timely, accurate documentation (OASIS, SOC/Admissions, Recertifications, ABNs, etc.).
• Follow safety regulations and report hazardous activities.
• Maintain current license, certifications, and clinical records as required.
