Job summary

Caringhand Home Health Solutions, Inc. in California is seeking an Occupational Therapist to help patients regain independence in their homes.

You will evaluate functional abilities, develop treatment plans, and teach daily living skills to support safe, effective self-care. Requirements include a CA OT license, 2 years of OT experience, CPR certification, a valid driver's license and reliable vehicle; willingness to travel to patient homes; and strong communication and organizational skills.

Qualifications

• Graduate of an occupational therapy curriculum accredited by ACOTE and AOTA.
• Registered to practice with the supporting OT credentialing body.
• Active California OT license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license and reliable insured vehicle; ability to travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in activities of daily living.
• Recommend adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and report changes in patient condition.
• Coordinate with the broader care team for patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.
