Job summary

VNA OF HANOVER & SPRING GROVE is seeking a Home Health Occupational Therapist to deliver patient-centered care in patients' homes across Adams and parts of York County. The role emphasizes independence, safety, and functional goals, with a focus on ADLs/IADLs and caregiver education.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license in the state of practice.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current automobile insurance.
• Strong communication, organizational, and clinical assessment skills.

Responsibilities

• Perform comprehensive occupational therapy evaluations in patients' homes.
• Develop individualized treatment plans based on physician orders and patient goals.
• Provide skilled interventions to improve ADLs and IADLs.
• Assess home safety and recommend adaptive equipment and environmental modifications.
• Train patients and caregivers in energy conservation, fall prevention, and compensatory strategies.
• Collaborate with nurses, physical therapists, speech therapists, social workers, physicians, and caregivers to coordinate patient care.
• Monitor patient progress and adjust treatment plans as appropriate.
• Complete timely and accurate electronic documentation.
• Participate in interdisciplinary team meetings and case conferences.
• Ensure compliance with all agency policies, payer requirements, and regulatory standards.
