Job summary

VNA of Hanover & Spring Grove is seeking a compassionate Home Health Occupational Therapist to help patients regain independence in their homes. You will provide skilled therapy, education to caregivers, and personalized treatment plans to support recovery and quality of life across Adams County and parts of York County.

You will work with an interdisciplinary team under the Clinical Manager, travel within the designated territory, and ensure compliant documentation and care coordination.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license in the state of practice.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current automobile insurance.
• Strong communication, organizational, and clinical assessment skills.

Responsibilities

• Perform comprehensive occupational therapy evaluations in patients' homes.
• Develop individualized treatment plans based on physician orders and patient goals.
• Provide skilled interventions to improve ADLs and IADLs.
• Assess home safety and recommend adaptive equipment and environmental modifications.
• Train patients and caregivers in energy conservation, fall prevention, and compensatory strategies.
• Collaborate with nurses, therapists, social workers, physicians, and caregivers to coordinate patient care.
• Monitor patient progress and adjust treatment plans as appropriate.
• Complete timely and accurate electronic documentation.
• Participate in interdisciplinary team meetings and case conferences.
• Ensure compliance with all agency policies, payer requirements, and regulatory standards.

What We're Looking ForRequired Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current Occupational Therapist license in the state of practice.
• Current CPR certification.

• Strong communication, organizational, and clinical assessment skills.

Preferred Qualifications

• One or more years of experience as an Occupational Therapist.
• Home health experience preferred.
• Experience working with adult and geriatric populations.
• Knowledge of Medicare home health regulations and documentation requirements.
