Job summary

UnitedHealth Group is seeking a Home Health Occupational Therapist (OT, OTR/L) for full-time, part-time, or PRN positions. The role covers assessment of functional status, ADLs, fine motor skills, home assessments, and adaptive equipment within a home health setting.

You will work with a multidisciplinary team to develop and implement care plans tailored to each patient. Primary responsibilities include evaluating patients, coordinating with physicians, and documenting progress while providing

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and access to a dependable vehicle or public transportation.
• Ability to function in any home situation regardless of the client characteristics.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy and in coordination with the health care team.
• Evaluates patient functional status and occupational therapy needs, and consults with physician on plan of care.
• Observes, records, and reports patient responses to treatment and changes in condition to supervisor/physician.
• Conducts patient assessments evaluating the level of function using diagnostic and prognostic tests.
