Job summary

UnitedHealth Group's Assured Home Health is seeking an Occupational Therapist in Home Health to assess and evaluate patient care needs, functional status, ADLs, and home assessments. You will support plan of care decisions with evidence-based techniques and coordinate with the health care team.

Responsibilities include documenting patient responses, conducting functional assessments, and applying adaptive equipment as needed. Strong communication and independence are essential in this role.

Qualifications

• Current Occupational Therapy licensure in state of Washington.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or able to obtain within 90 days.

Responsibilities

• Evaluates patient's functional status and CMS-required domains to inform plan of care.
• Provides OT services within scope of practice and collaborates with health care team.
• Documents responses to treatment and patient condition changes; conducts home assessments.
• Ensures care aligns with CMS guidelines and physician-approved plans of care.
