Job summary

Qualifications

• Licensure to practice Occupational Therapy in WI and/or IL.
• NBCOT registration or eligibility for certification and registration.

Responsibilities

• Reviews physician's referral, patient's condition and records to determine OT treatment.
• Evaluates range of motion, strength, balance, and ADL/IADL status in home setting.
• Develops goals and a home-based treatment plan to restore function and independence.
• Implements therapeutic activities to restore, reinforce, and enhance performance.
• Instructs patients and families in home programs and use of adaptive equipment.
