Beacon Home Health in Smithville, TN seeks an Occupational Therapist to join our Home Health team. You will assess patients’ functional status, ADLs, and need for adaptive equipment, delivering care per plan and state regulations.

Responsibilities include evaluating CMS-related status, developing therapy plans with physicians, and documenting progress for continuity of care. Must hold an OT license in TN and CPR certification; reliable commute required.

Competenze

• Current OT license in the state where services are rendered.
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

• Assess and evaluate patient care needs related to functional status, ADLs, fine motor coordination, home assessments and adaptive equipment, and other OT needs as defined by medical conditions.
• Provide service within the scope of practice as defined by state laws, in accordance with the plan of care, using evidence-based techniques, and in coordination with the health care team.
• Evaluate patient’s functional status and CMS documentation; consult with the physician in the development of the therapy plan of care.
• Observe, record, and report to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.
• Conduct patient assessments evaluating level of function by applying diagnostic and prognostic functional ability tests.
