Job summary

Beacon Home Health in Lebanon, TN is seeking a Certified Occupational Therapist Assistant (COTA) to join our Home Health team. The COTA will provide in-home OT services, follow the plan of care, and work closely with the supervising OT and health team to support patients’ functional independence.

Position requires current state license and CPR, reliable transportation, and ability to travel between patient residences daily.

Qualifications

• Current license in the state where services are rendered.
• Current CPR certification.
• Driver's license and access to reliable vehicle.
• Must be able to commute to multiple patient residences daily.

Responsibilities

• Provide occupational therapy to patients in their homes to restore them to their fullest physical ability.
• Follow the plan of care and instruct patients in evidence-based treatment within the scope of an OTA and state laws.
• Observe, record, and report patient progress to the supervising OT, nurse supervisor, or physician.
• Coordinate care with other health care team members as appropriate.
• Instruct patients, families, caregivers, and others on therapy areas within the OTA scope.

The COTA performs his/her duties in accordance with policies and procedures and the established plan of care. A few position highlights include:Provide service within the scope of practice as defined by the state laws governing the provision of Occupational Therapy in home care, and in accordance with the plan of care set only as defined by the supervising OT.Follow the plan of care, instruct and aid patients in evidence-based treatment within the scope of the occupational therapy assistant and according to the state laws governing occupational therapy assistants.Observe, record, and report to the supervising OT, the nurse supervisor, and/or the physician the patient's response to treatment and changes in the patient's condition.
